Admission requirements

• To better serve, prior to accepting any resident for admission into Harbourview Care Center, an evaluation of the potential resident's medical and social needs must be completed by our admissions team. Please arrange access to medical records by signing a release form with the physician and/or current hospital or place of residence.

In addition, the following is required prior to admission:

• Medicare/Medicaid and /or private insurance cards must be available for us to copy and verify.
• Name, address and contact numbers of responsible party and one other person. Choice of doctor (medical, dentist, podiatrist, etc.)
• Choice of pharmacy that follows facility and state guidelines.
• Pay 30 days in advance for private pay and Medicaid pending.
• Copies of DNR/Living Will.
• Copies of POA/Guardianship.
• All personal items must be marked, labeled or etched.
